- Manage a client caseload within the outpatient behavioral health clinic.
- Conduct intake assessments, including biopsychosocial evaluations.
- Collaboratively develop, implement, and update individualized treatment plans and goals.
- Provide individual, family, and group counseling using evidence-based and culturally responsive practices.
- Facilitate therapeutic groups and psychoeducation sessions.
- Document all clinical encounters, treatment progress, and case management notes in the electronic health record in a timely and compliant manner.
- Provide crisis intervention and stabilization support appropriate to an outpatient setting.
- Coordinate care and make referrals to psychiatry, primary care, urgent care, social services, and other community resources as needed.
- Provide and receive warm handoffs between urgent care, primary care, and behavioral health services within Transformation Health Network to support continuity of care and whole-person treatment.
- Collaborate with schools, courts, probation/parole, and other external partners when client consent or court order applies.
- Attending team meetings, interdisciplinary case conferences, and clinical supervision.
- Engage in regular training and professional development to support high-quality, whole-person care.
- Perform other related duties as assigned or needed to support clinic operations.
